2007/2008
Quarterfinals & Semi-Finals
All
Quarter Finalist teams must send a team representative to check in their
team credentials 1 ½ hours prior to the start of their first game.
Teams must check in at the tournament headquarters in the office at
Morgan Hill Soccer Complex on Saturday, April 26th or Sunday
April 27th according to the following schedule:

Teams
with 8:30 games must check in at 7:00 AM
|

Teams
with 10:30 games must check in at 9:00 AM
|
|
Teams
with 12:30 games must check in at 11:00 AM
|
|
Teams
with 2:30 games must check in at 1:00 PM
|
|
Teams
with 4:30 games must check in at 3:00 PM
|
Teams
playing Saturday that do not advance to semi finals should collect their
credentials at the headquarters following their game.
The
credentials for teams advancing to the semi final matches on Sunday will
be retained by the CYSA Registration Committee Chairman.
On Sunday,
U14 through U18 semi finalist teams should report directly to their
assigned field for the pre-game team check in with the referees. Under
19 semi finalist teams must send a team representative to check in their
credentials as noted in the first paragraph.
Check in requirements will be the
same as for previous rounds. In addition, the items listed below are
required at the time of Quarter Finals check-in.

When your team is checked in, CYSA will keep the team binder, along
with the US Youth Soccer member passes for each player and
coach/registered team official. The binder should contain only CYSA
related paperwork and should be organized as follows:

Official CYSA team roster
(Goldenrod)

Player 1601 forms in the order
the players are listed on the goldenrod

Second copy of Player 1601 forms
with wet ink signature. The 1601 forms are available or download on
the CYSA website at
www.cysanorth.org. Click on Forms & Manuals->Registration
Forms. The second copy of the 1601 form can
be a photocopy of the existing form so long as you get a wet ink
signature on the photocopied form. Also note, if any player on your
roster has turned 18 since the first 1601 form was signed, you will
need to have the player sign the original 1601 as well as the
photocopy of the 1601.

Each team must provide three game card stickers containing all
required information (players’ names, jersey numbers, and CYSA
identification numbers). A template for producing your game card
stickers can be found on the CYSA website at
www.cysanorth.org. Click on Forms & Manuals, then under “CYSA
Forms” click on “game card stickers.” Complete the template to
produce a document that fits the game card by using Avery Sticker
5168.

This is ONLY for U17-U19 State Cup Play

A 1
½” x 1 ½” picture of each player and up to four registered team
officials who will be attending Far West Regionals should the team
advance, with the name of the individual on the back, must be
submitted. Each picture must be a clear, current photo – head and
shoulders –
NO POLAROIDS! NO HATS! NO SUNGLASSES!

Binders will be kept by CYSA until the team is eliminated. Teams are
responsible for picking up their binders after they are eliminated.
