Notice
Note: 2008/2009 LEAGUE AFFILIATION
AGREEMENT (Form 8201) MUST be signed by
two (2) League Officials and submitted with a League check for $100.00 payable to CYSA.

A copy of the League's current
Operating Procedures (Constitution, Bylaws, General Rules).

League's
MUST submit current proof of "Good Standing"
with the Internal Revenue Service (IRS) and Secretary of State.

Leagues will be
required to have their own D&O (Directors & Officers) insurance coverage
in the amount of at leagues $1 million as a condition to be affiliation
with California Youth Soccer Association, Inc. A copy of D&O
insurance must be provided with this 2008/2009 League Affiliation
Agreement Form.

This
agreement and the affiliation represented by it shall become effective
only when signed, and approved by, California Youth Soccer
Association, Inc. State Board of Directors.
By filling out this form and submitting your
payment does not guarantee League Affiliation with California Youth
Soccer Association until the CYSA State Board of Directors has signed
and approved affiliation

leagues, their teams and players, shall abide by the Constitution and
Bylaws of the California Youth Soccer Association, inc., the United
States
Youth Soccer
Association, and the United States Soccer Federation; All General
Procedures and Specific Rules are set forth by the Board of Directors
and all application rules and regulations of the United States Youth
Soccer Association and the United States Soccer Federation.
