the application deadline for CYSA FOUNDERS’ cup u12 & u14 is november 1,
2007.
TEams may apply either by downloading an application form on this page and
submitting with the required components or by using the online method. in
order to apply online, you must read and agree to the following important
information:
All OF THE required steps for the CYSA FOUNDERS’ Cup application process
must be completed FOR u12 AND u14 TEAMS no later than 5:00 PM ON
thursDAY,
November 1st, 2007 in order for your application to be considered complete.
IF YOU ELECT TO APPLY USING THE ON-LINE PROCESS, In addition to completing
the online application, you must SUBMIT THE FOLLOWING ITEMS ALONG WITH A
COPY OF THE PRINTED ON LINE APPLICATION RECEIPT:

a copy of your official CYSA team roster that has been
STAMPED by your district registrar. YOU MAY SUBMIT THE COPY OF THE
ROSTER VIA FAX, BY SCANNING AND EMAILING, OR THROUGH OTHER DELIVERY
OPTIONS (US POSTAL SERVICE, FED-EX, UPS, ETC.)

if you chose not to pay THE entry fee by visa or
mastercard using the on-line process, you must either submit a check or
money order in the amount of $300 payable to CYSA or call the cysa
office to arrange for payment on either a visa or mastercard.
ALL STEPS OF THIS PROCESS MUST BE COMPLETED AND IN THE CYSA STATE OFFICE FOR
U12 AND U14 TEAMS NO LATER THAN 5:00 pm NOVEMBER 1st, 2007 IN ORDER FOR YOUR
APPLICATION TO BE CONSIDERED COMPLETE. LATE RECEIPT OF ANY OF THE REQUIRED
ITEMS FOR ANY REASON WILL RESULT IN THE REJECTION OF YOUR FOUNDERS’ CUP
APPLICATION. NO EXCEPTIONS!!!
