ASSOCIATION CUP TEAM CREDENTIALS CHECK IN INFORMATION
The
Friday Night Credentials Check-In is mandatory for all participating
teams unless approved for a game-day check in by the Association
Cup Committee Representative from the District in which your Association
Cup games will take place. If a representative from your team cannot
attend the scheduled Friday night Check-In, you must be pre-approved for
alternate check in arrangements by the hosting district’s Committee
Representative. Teams approved for a game-day credentials check must
check in with the venue coordinator no later than 1 ½ hours prior to the
start of their first scheduled match. Teams should be aware that
credentials check the day of the event can severely limit their ability
to correct any credentials deficiencies that could have been discovered
during the Friday night credentials check process. Player and/or team
disqualifications could result from deficiencies that cannot be
corrected in time for the first scheduled match. Failure to give Cup
officials adequate time to perform a thorough credentials check could
cause a delay in the start of your match, and could ultimately result in
the forfeiture of your teams matches.

The team’s
official 2007/2008 CYSA team roster that has been stamped by the
team’s district registrar which includes any added, transferred and
released players and. |

A 1601
membership form for each player registered to your team with the
consent for medical treatment and the hold harmless statement signed
by the player’s parent or guardian in original ink. The 1601 forms
must be arranged in order the players are listed on the roster.

A proper US
Youth Soccer 2007/2008 member pass which has been stamped by the
team’s district registrar. There must be a member pass for each
registered team official and each player registered to the team.
Each member pass must be laminated. |

Each team must
provide 3 game card sticker containing all required information
(players’ names, jersey numbers, and 12 character CYSA identification
numbers.) A template for producing your game card stickers can be
found on the CYSA website at
www.cysanorth.org. Click on Forms & Manuals, then under “CYSA
Forms” click on “game card stickers.” Complete the template to
produce a document that fits the game card by using Avery Sticker
5168. |
